Support Coordination
"Helping you understand your NDIS plan, connect with the right supports and have more control over how they work together."
What is Support Coordination?
Support Coordination helps you understand your NDIS plan and make the most of the supports available to you.
A Support Coordinator can help you find and connect with providers, organise your supports, understand your options and work through problems when things are not going as planned.
Over time, the aim is to help you build the confidence and skills to make informed decisions and manage more of your NDIS supports yourself, where possible.
How can a Support Coordinator help?
Depending on what you need, your Support Coordinator may help you to:
- understand your NDIS plan, funding and supports
- find and compare providers that suit your needs and preferences
- connect with NDIS, health, community and other services
- organise your supports and help different services work together
- understand service agreements and what you are agreeing to
- keep track of how your plan is being used and whether your supports are working for you
- prepare for NDIS meetings or changes to your plan
- work through gaps, changes or problems with your supports
- explore other options if a provider or support is not working for you
- plan for times when your usual supports may not be available
- build your confidence and skills to manage more of your NDIS supports over time.
Your Support Coordinator can explain your options and help you work through decisions, but the choices remain yours.
How we work with you
We take time to understand what matters to you, what you need help with and how you would like your supports to work.
We provide clear information, talk through your options and follow through on what we agree to do. You should feel comfortable asking questions, raising concerns or telling us when something is not working.
You remain in control of your supports and the providers you choose.
If you want other people involved, we can also work alongside your family, friends, carers, advocates or other services to help keep your supports connected and everyone clear about what is happening.
Support Coordination and Recovery Coaching
Support Coordination and Recovery Coaching can help with some of the same things, but they have different areas of focus.
Support Coordination helps you understand and use your NDIS plan, connect with providers, organise your supports and build your confidence to manage them.
Recovery Coaching is specifically for people living with psychosocial disability and has a stronger focus on personal recovery, managing everyday challenges, and building skills and independence.
Most participants will have funding for one or the other. If your NDIS plan includes both, their roles should be clear so they can work together without duplicating the same support.
Support Coordination and Plan Management
Support Coordination and Plan Management are also different.
A Support Coordinator helps you understand and organise your NDIS supports, connect with providers and work through issues.
A Plan Manager looks after the financial side of your NDIS funding, such as paying provider invoices and helping you keep track of spending.
Some participants have both.
Funding and getting started
Support Coordination needs to be funded through your NDIS plan.
If you’re not sure whether your plan includes Support Coordination, we can help you understand what is available.
