My ResponsAbility

Hospital-to-Community Transition Support

"Practical support to help you prepare for life after hospital and get the right disability supports in place."

What is Hospital-to-Community Transition Support?

Leaving hospital can involve a lot of changes at once.

You may be returning home, moving somewhere new, starting with new support workers or trying to work out how different services will fit together once you leave.

My ResponsAbility can work alongside you before and after discharge to help organise the disability supports you need, make the transition easier to understand and help put practical arrangements in place.

How can we support your transition?

Depending on what you need and the supports funded in your NDIS plan, we may help you to:

  • take part in discharge or transition meetings with you
  • understand what NDIS supports will be needed when you leave hospital
  • communicate with the hospital, your NDIS contacts and other services involved in your transition
  • find and organise suitable disability support providers
  • arrange support workers and prepare them for your support needs
  • help make sure relevant plans, instructions and support information are understood before support begins

If you or someone supporting you is unsure what can be provided, we can talk through what support is needed and whether My ResponsAbility may be able to assist.

You can refer yourself, make a referral with someone you trust, or ask your hospital team, Support Coordinator or another service to refer you.

My ResponsAbility supports participants across the Sydney metropolitan area.

If you are preparing to leave hospital, tell us what support you need and where things are up to.

This service is currently available, subject to participant needs, funding, location, risk, suitability and workforce capacity.

Make a Referral or Contact Us to talk it through first. You can also call 02 7205 4859.
