Accommodation, Tenancy & Life Transitions
"Practical support to navigate changes in where you live and prepare for what comes next."
What does this support involve?
This support is for people who need practical help to navigate changes in where or how they live.
You might be exploring different living options, preparing to move, settling into a new home or dealing with changes that are making your current living situation harder to manage.
My ResponsAbility can help you understand your options, organise disability supports, prepare for changes and connect with the right housing, tenancy or community services where needed.
The focus is on the practical and disability-related parts of accommodation and life transitions. My ResponsAbility does not provide the home or accommodation itself.
This might be right for you if…
You may be looking for this kind of support if:
- you’re leaving hospital or another setting and need help working out what comes next
- your current living situation is becoming difficult to manage
- you’re preparing to move out of the family home for the first time
- you’re moving to a new home or support arrangement
- problems with your tenancy or shared living situation are affecting your stability
- you need help understanding your options or connecting with the right services.
How can we support you?
Depending on what you need, we may support you to:
- explore different living options and understand what may suit you
- identify the disability supports you may need at home
- break down the practical steps involved in preparing for a move
- connect with relevant NDIS, housing and community services
- prepare for meetings about your housing or support needs
- communicate with providers and other services involved in your transition
- build skills and routines for managing everyday life at home
- settle into a new home, neighbourhood or support arrangement
- work through changes or problems affecting the stability of your living situation
- connect with specialist housing, tenancy or advocacy services where needed.
The support you receive will depend on your circumstances, NDIS goals and available funding.
How we work with you
Where you live can affect many parts of everyday life, so we take time to understand what matters to you and what you would like to change.
We provide clear information, help break bigger tasks into manageable steps and support you to be involved in decisions about your home and supports.
If you want other people involved, we can also work alongside your family, friends, carers, Support Coordinator, housing provider or other services to help keep things organised and connected.
You remain at the centre of decisions about where you live and how you want to be supported.
Understanding the role of this support
My ResponsAbility can support you with the practical and disability-related parts of exploring, preparing for or adjusting to a living arrangement.
We do not provide housing directly or guarantee that accommodation or a housing application will be successful.
We also do not provide legal tenancy advice. If you need specialist housing, tenancy, advocacy or legal assistance, we can help you connect with an appropriate service.
NDIS funding generally covers disability-related supports rather than everyday living costs such as rent, groceries and household bills. Some participants may also have specific home and living supports included in their NDIS plan.
Funding and getting started
The support we can provide depends on your NDIS plan, goals and the type of assistance you need.
If you’re unsure whether your funding can be used for this support, we can help you understand what is available.
You can refer yourself, make a referral with a family member, friend or someone you trust, or ask another person or service to refer you.
My ResponsAbility supports participants across the Sydney metropolitan area.
Tell us what’s changing and what you need help with, and we’ll take it from there.
